How do I create a 360 Feedback Template?

First, you must navigate to Admin > Performance > Feedback, here you will be greeted with a blank page (assuming you have not created any templates yet). Clicking new will bring up this menu:

You must add a Title to the template, but a description is optional. Once you fill out what is needed, you can click Create. Once you do so, you will be brought to another blank page where you can add content:

Click the "Add Content" button, here you will be presented with a few different types of content you can add to the template.

You have the option to add Text, Text Questions, Rating Scale, or a Dropdown menu.

The Text option is a simple text box where you can add anything you want to address in the Template.

The Text Question option allows you to add a block for a question, similar to the course builder. You can ask a question, and the assignee will then answer with their own text.

Rating Scale allows you to add a sliding scale, just like the course builder, where you can ask a question that the assignee will have to answer by sliding the scale left or right. Clicking the Gear icon will bring the settings for the scale up. Here, you can change the number of rating steps and also provide custom labels per number.

Lastly, the Drop Down option allows you to create a Drop Down menu for the assignee to choose from. You can also choose to have multiple options/multiple answers.

Once you are satisfied with your feedback template, clicking the arrow to the top left will take you back to the Admin Feedback main menu, and you will see your new template in the overview.
